Form 1095-A, Health Insurance Marketplace Statement, is the document the Marketplace sends you each January if anyone in your household had a Marketplace plan the previous year, and you use it to reconcile your Premium Tax Credit on IRS Form 8962 when you file taxes, per IRS (2026).
